We look forward to the opportunity to serve your health care needs. Our goal is to treat you in a respectful and responsive manner. Our physicians have excellent credentials, and we seek to care for all of our patients in a compassionate, friendly atmosphere.

If you have an HMO type of insurance, you will probably need to obtain a referral to our specialist office. If you have lab tests performed in the weeks preceding your appointment with us, please try to bring a copy of the results with you. We normally take care of registration and co-payments as patients check-in so we can focus the end of the visit on clinical issues. We accept checks, cash, and most credit cards.

New Patient New Patient Information
  • Please bring a list of all current medications to each office visit.
  • Please bring or have your referring physician or family doctor fax any relevant medical records pertaining to your visit (our fax number is 856-222-0721).
  • Please bring insurance cards and any referrals needed.
  • We usually need a urine specimen before you are seen by the doctor. It may help not to use the restroom before the start of your visit.

Medical Insurance Medical Insurance Information

Every effort is made to process claims to your insurance company efficiently and accurately.  Please make sure that we have all the correct information in our records.

We need to know:

  • Patient name, address, telephone number, social security number, cell phone number, email address, and date of birth.
  • Insurance Company name, address and telephone number.
  • Insurance Plan name and number
  • Policyholder name, date of birth, social security number, relationship to patient, and policyholder’s employer name and address.
  • If you have more than one insurance coverage, the practice is obligated to determine which plan is primary.
  • Please be prepared to present your driver’s license or other form of identification along with your health insurance card.
  • We do not participate in all managed care insurance programs.  Please speak with our billing staff if there is any question about your plan. Click Here to review a list of insurance companies with whom we are considered participating providers.
  • Copayments and any past due balances are due when you check in. We accept cash, checks, VISA, MasterCard, Discover Card, and American Express.
    Patients with special circumstances should call the billing department at (856) 222-1975, option 5.

Your Appointment Your Appointment
  • Your time is important to us.
  • Patients are seen in order of scheduled appointments.
  • Some of our patients will need more time with the physician than they are allotted - please be patient. You will receive the same consideration.
  • When you are finished with your visit, we can usually set you up for your follow-up appointment. If needed, we will mail you a card at the time when you are due for your next visit. You should promptly call the office at that time to make your appointment.
  • If you do not receive a card to call for an appointment at least one month before our doctor wanted to see you again, please call the office.

Other Information Other Information

  • We ask our patients to provide at least 24 hours notice if you will be unable to keep an appointment. We would like the opportunity to make the time available to another patient. Patients who do not give us a 24-hour notice for a canceled or rescheduled appointment or who do not show up will be charged an additional $50.
  • Prescription refills normally require 48 hours advance notice.
